We use the information we collect to provide, maintain, and improve our services, including to process your telehealth requests and communicate with you.
3. Medical Information
Any medical information shared during consultations or through our website is kept strictly confidential in accordance with applicable medical privacy standards.
4. Contact Form, Security and Analytics
By continuing to use this website, you accept our collection of the information necessary to provide our services. When you use the contact form we receive your name, email address, optional phone number and message, and we send a confirmation to the email address you provide. If you answer the optional questions about how you heard about us and what you searched for, we also receive those answers. To protect the form from abuse we keep a one way, non reversible fingerprint (hash) of your IP address and email address for a short time. We use Google Analytics to understand how the website is used.
5. Data Retention
An automatic cleanup runs once a day and deletes anti abuse records older than 2 days and email delivery records older than 90 days. Messages you send through the contact form are delivered to our practice inbox and are not kept in the website database once delivered. Your answer to the optional "How did you hear about us?" question (the option you choose, not any text you type) is stored separately, with only the site language and the date, without your name, email, phone number, message or IP address, to help us understand how patients find us, and is deleted after 400 days. Anything you type about what you searched for is only included in the message delivered to our inbox.
