Vitiligo and psoriasis: long term conditions, real options
Both conditions are lifelong and both are far more treatable than they were a decade ago. What to expect from each stage.

Vitiligo and psoriasis are immune-driven conditions that come and go over years. Neither is contagious, neither is caused by anything the patient did, and both carry a social weight in our community that is often heavier than the physical symptoms. Treatment aims for control, comfort and confidence rather than a single cure.
Key points
- Early vitiligo treatment gives the best repigmentation
- White patches burn easily and need sun protection
- Scalp psoriasis needs scalp-specific products
- Biologic treatment has transformed severe psoriasis
- Joint pain with psoriasis needs assessment
Vitiligo: what treatment can achieve
Vitiligo is loss of pigment cells, producing sharply defined white patches. Repigmentation is most successful on the face and neck, slower on the hands and feet, and best when treatment starts early while patches are still active.
Options include topical steroids and calcineurin inhibitors, narrowband ultraviolet B phototherapy, and newer topical JAK inhibitors where available. Sun protection of the white patches is essential because they burn easily. Camouflage cosmetics are a legitimate part of care, not a failure of treatment.
Psoriasis: from creams to biologics
Mild plaque psoriasis is treated with vitamin D analogues, topical steroids and coal tar or salicylic preparations for thick scale, with emollients throughout. Scalp psoriasis needs a medicated shampoo plus a scalp solution rather than a body cream.
Moderate and severe disease has changed completely. Phototherapy, methotrexate, ciclosporin and acitretin remain useful, and biologic medicines now clear the skin in a majority of patients who need them. Psoriasis also affects the joints in a proportion of patients, so persistent joint pain or morning stiffness should always be reported.
The whole person, not just the skin
Psoriasis is associated with metabolic syndrome, diabetes, high blood pressure and cardiovascular risk, so blood pressure, weight, lipids and sugar are reviewed as part of care. Smoking and heavy alcohol worsen it. Both conditions are strongly linked to anxiety and low mood, and it is reasonable to treat that alongside the skin.
Planning long term care
A useful plan sets a treatment for flares, a maintenance treatment between them, a review interval, and clear warning signs that should prompt earlier contact. Patients living outside Baghdad can hold most reviews by video and attend in person for phototherapy or when a change of systemic treatment needs monitoring.
Frequently asked questions
Is vitiligo related to thyroid disease?
Vitiligo is associated with autoimmune thyroid disease, so thyroid function is usually checked at diagnosis and periodically afterwards.
Can psoriasis be cured?
There is no permanent cure, but long periods of clear skin are realistic with modern treatment. The aim is sustained control with the least burdensome regimen.
