Microneedling with mesotherapy in Baghdad and Karbala: what the combination can and cannot do

Microneedling has good evidence for acne scars and helps stubborn melasma alongside creams. What is applied during the session matters: it must be sterile and made for injection, and its own evidence is often limited.

Microneedling makes tiny, controlled channels in the skin, which then heals and remodels. In this combined treatment a solution is applied during the session, so some of it passes through those channels into the skin, as in mesotherapy. The needling part has the stronger evidence, for depressed acne scars [1]. The evidence for the applied solution depends entirely on what it is, so we tell you which part of the result is well supported and which is not.

    Key points

    • Microneedling itself can improve the texture of acne scars over several sessions
    • Adding PRP may give somewhat better acne scar results than microneedling alone
    • For melasma, microneedling with creams or tranexamic acid is an add-on for stubborn cases, with short-term evidence
    • A licensed hyaluronic acid skin booster may improve hydration and glow for a few months
    • Only sterile products made for injection should ever be applied through the channels

    What each part contributes

Microneedling: a review of 9 randomised trials found it effective for atrophic acne scars, alone or combined with other treatments, with no serious side effects reported [1]. Adding platelet rich plasma (PRP) may give somewhat better acne scar results than microneedling alone [3].

Melasma: a meta-analysis of 12 studies found that adding microneedling to topical treatment gave extra improvement at 8 and 12 to 16 weeks, with short follow-up and no data on relapse [4]. A 2022 review of 33 studies of mesotherapy for melasma, most of them using tranexamic acid, concluded that it can be an alternative or add-on treatment and that larger studies are needed [5]. Either way, it sits inside a melasma plan with prescription creams and sunscreen.

Skin quality: licensed hyaluronic acid skin boosters have a little data, for example a randomised split face study in 55 women that found better elasticity and radiance than saline [6]. A fair description is that they may improve hydration and glow for a few months. For other aesthetic mesotherapy mixtures, France's health authority (HAS) found no positive clinical evidence [7].

What may be applied, and what should not

Anything applied during needling can reach the deeper skin, so it is effectively injected. A 2026 mesotherapy consensus recommends using products made for injection (others count as off-label) and strict sterile technique [8]. HAS documented infections, including with atypical mycobacteria, and risks from non-standardised products [7].

    • Appropriate: sterile products licensed for injection into the skin, such as a hyaluronic acid skin booster, chosen for your concern
    • Not appropriate: cosmetic serums, unlabelled mixtures or anything that is not sterile and made for injection
    • Ask for the product name and its licence before treatment

Darker skin and who should wait

Darker skin can develop dark marks after inflammation [2]. Dark marks after microneedling were rarely reported in the acne scar trials [1], and because microneedling leaves the skin surface largely intact, dermatologists generally consider it lower risk for dark marks than many lasers. That comparison is clinical reasoning, not a finding of the review. Strict sun protection before and after treatment still matters, especially during Iraq's long, intense summers.

We postpone treatment if the skin is infected or actively inflamed, and we discuss acne medicines, other health conditions and pregnancy at the consultation.

    Frequently asked questions

How is this different from microneedling alone?

The needling is the same. The difference is a sterile solution applied during the session. The needling has the stronger evidence; the benefit of the solution depends on what it is.

Is it safe for brown skin?

Microneedling is generally a good option for darker skin, as dark marks were rarely reported in trials. Sun protection before and after treatment is still essential.

    Can it treat melasma?

It can be added to prescription creams for stubborn melasma, with good short-term results in studies. It does not replace creams, tinted sunscreen or maintenance treatment.

Can I use my own serum?

No. Only sterile products made for injection should go through the channels. Cosmetic serums are made for the skin surface.
